The Hybrid Measure Reboot:
What’s Changed, What’s Next, and Why It Matters
Thursday, July 31, 2025 1:00 PM Eastern Daylight Time 1 hour
July 1 marked a major milestone: Hybrid measures are back on the mandatory reporting table and this time, CMS is all in. With the latest updates to the Hybrid Hospital-Wide Readmission (HWR) and Mortality (HWM) measures, hospitals must navigate a maze of revised specifications, CCDE rules, mapping requirements, and new risk adjustment inputs. From shifting timing rules to payer changes, this session will break down what’s new, what’s critical, and what teams must do now to stay compliant and avoid penalties. Plus, we’ll explore how Hybrid measures are positioned to play a starring role in CMS’s new TEAM Model and what that means for your organization’s reporting strategy.
Whether you're back at the table after sitting out during the voluntary phase or trying to refine your current workflows, this session will help you align your strategy, tools, and teams for Hybrid success.
By the end of the webinar, attendees will be able to:
- Explain the evolution and current status of Hybrid HWR and HWM measures, including the 2025–2027 reporting requirements.
- Identify the key changes in measure specifications, linking variables, and CCDE data capture across the latest three versions.
- Understand CMS’s fallback logic for non-standard units and how it can impact hospital performance.
- Evaluate EHR workflow and mapping readiness for capturing required clinical data in alignment with CMS specs.
- Recognize the growing importance of Hybrid measures in future models, such as TEAM, and the implications for cross-functional quality strategies.
- Take action on critical to-dos, including provider education, mapping validation, and structured documentation setup.
Attendees will receive 1 CPHQ Continuing Education Credit
for participating live.
