Quality Is the Gateway to Cost Control: What TEAM and CJR-X Signal for Hospitals

CMS is expanding its use of mandatory payment models that connect quality performance, episode spending, and financial accountability.

TEAM is already creating new responsibilities for selected hospitals. The model runs from 2026 through 2030, and for many participants, 2026 serves as a one-year glide path before downside risk begins in 2027. Hospitals need to use this time to understand their financial exposure, establish cross-functional ownership, and determine how they will monitor performance before final reconciliation.

CJR-X extends this direction. Beginning in 2028, most eligible hospitals nationwide will be accountable for the quality and cost of lower-extremity joint replacement episodes. Taken together, TEAM and CJR-X signal that CMS is prepared to scale episode-based accountability when high-volume, high-cost areas demonstrate the potential for broader application.
