Hybrid measures: voluntary today, required tomorrow.

This year starts the voluntary reporting period for the Hybrid Hospital-Wide 30-Day Readmission (HWR) measure which becomes mandatory in two years. Your performance is publicly reported and if you don’t submit the measure, you can lose a portion of your Medicare reimbursements.

  • A hybrid measure combines your claims data with electronic clinical data pulled directly from your EHR.
  • CMS combines these two files using linking variables (that you provide to CMS) to risk adjust your patient population.
  • That risk adjustment (based on your clinical and claims data) determines your performance on the Hybrid Hospital-wide 30-day Readmission Measure.
  • You are responsible for using the hybrid specification to map the measure’s value sets to your EHR system and submit an electronic file to CMS with specific information for them to process your results.
  • You won’t know your results until after CMS calculates your rate so you’ve got to be certain that your electronic file is perfect.
